When you tap on one of the piano keys, the corresponding note will be played (what else did you expect? :-) If you are in recording mode (the status display on the top right would the show "Record" instead of "Stopped", this note would also get added (or inserted) to the current track (see next picture). You determine the duration (e.g. 1/4) of the inserted note by choosing it from the duration checkbox (see picture left) before you tap on the keyboard. As an alternative, you can also choose to enter it it real time mode - PocketSynth Professional will then determine how long you depressed the key and use the appropriate duration value. An optical metronome (the rectangle on the lower left corner flashes) helps you to stay in sync.
For advanced rhythmic choices - arpeggios, strumming patterns, etc. - you can bring up the integrated pattern editor.
